Integrated Community Systems for Children/Youth with Special Healthcare Needs in California
The goals of this project are to develop and implement integrated, community-based, family-centered systems of care for C/YSHCN; promote family-centered care for C/YSHCN through family/professional partnerships and comprehensive health care for C/YSHCN through identified medical homes; promote effective transition to adult health care, work and independence; and ensure sustainability of systems of care post-project to meet the six core measures for children with special health care needs defined by the federal Maternal Child Health Bureau.
Project partners include Family Voices of California, Lucille Packard Childrens Hospital and the Children's Medical Services. A key stake holder informs and monitors project activities and project information is disseminated via an electronic newsletter, the CaCSHCNews. |
