CMHP California Medical Home Project
Funded by the California HealthCare Foundation, the California Medical Home Project (CMHP) aimed to improve services to children with special health care needs in CA by increasing the number, and improving the quality of medical homes to serve this vulnerable population.

The project was implemented through several components, including a Program Office, located at Childrens Hospital Los Angeles that provided technical assistance, project oversight and consultation to project participants; a Statewide Coalition, to address overarching policy issues related to service delivery for children with special needs; and seven locally based coalitions that developed strategies for improving service delivery in selected communities. Intervention strategies included provider and parent training about the system of care for children with special health care needs, development of parent notebooks to aid in organization of materials and sharing of information with providers, parent participation on advisory boards, and the development of educational materials for parents and professionals.

The project was evaluated by staff of the Center for Healthier Children, Families and Communities at the UCLA School of Public Health.
